A Clinical Study for the Treatment of Acute Flexor Tendon Injuries in zone II and of the Hand

Abstract
The treatment of flexor tendon injuries in Zone II is still somewhat controversial. In this zone, the superficialis and profundus tendons are held together closely in a common sheath and the fibro-osseous tunnel. The results of primary repair or
tendon
graft in this area were usually so poor that Zone II was designated as "No man's land" From November 1993 to October 1994, the 35 cases of acute of acute flexor tendon injuries in Zone II of the hand which were experienced at Pusan Choon Hae
Hospital
were studied.
The flexor tendons were treated with primary repair by core suture with modified Kessler method using 4-0 prolene and reinforced by epitendinous running suture using 6-0 prolene. Early controlled exercise using rubber band was started immediately
after
operation which was suggested by Young and Kleinert. The follow up period was ranged from 6 weeks to 9 months, the average length was 3 months.
@ES The results were as follows:
@EN 1. Most of the patients were injuried by knife(80%), and the most frequent finger of injury was index(38%).
2. Under the principle of primary repair and early controlled exercise, the range of motion of injured finger joint was recovered at 3 months after operation in an average.
3. The final result was rated excellent in 20 cases(58%), good in 8 cases(23%), fair in 4 cases(11%) and poor in 3 cases(8%) according to the criteria of kleinert.
4. There were several complications during follow up period which were adhesion of the repaired tendon in 3 cases(9%), re-rupture of the repaired tendon in 1 case(3%), delayed healing of the wound in 1 case(3%) and infecion in 1 case(3%).
